StartDate: 4/21/2025 Pay Rate: $2000.00 - $2700.00
POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 1+ year acute hospital case management experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00AM - 4:30PM M-F and every other weekend
SYSTEMS – Meditech, Midas, Wellsky
START DATE – ASAP
